Registration Form Instructions
PLEASE PRINT OUT THE FOLLOWING THREE (3)

THEN, You can either FAX them in with your
Credit Card information (IT IS SECURE!) to:
(305) 858-0535 (FAX NUMBER) or,
You can Mail them in to the address printed
on the forms.
(After June 15th, please either take them
in person to the UM Office,
or FAX your information to avoid a delay
and have to go through
LATE REGISTRATION.)
AFTER YOU FAX YOUR FORMS IN:
If you would like to find out if we have
received your forms IMMEDIATELY, please email us at efmmusco@aol.com
OTHERWISE,
You will receive a card in the U.S. Mail from the camp office letting you know that all has been received, or, if there is any balance due, and other information for you regarding the
camp. This process takes about TWO to THREE weeks.
If it is near to the Camp Starting Date,
you will be called or emailed.
